On the waterfront, Tabitha got excited when she saw Endora. She begged the forces to let her take Endora home. As the apparition gained shape, however, Tabitha realized it wasn't Endora. Instead it was Timmy, who handed Tabitha a Martimmy. After trying to grab his hand, Tabitha concluded it wasn't really Timmy either and demanded an explanation. Timmy insisted he was real, just in doll form. He said he came to warn Tabitha that if she continues her wicked ways she may never see Endora again. Tabitha said the dark forces are blackmailing her to do evil to get Endora and Miguel back. Timmy insisted that doing evil is what's preventing them from returning, and declared that "goodness is the way" before disappearing.

Pretty laid back and marveled at Luis while he made love to her. They were rudely interrupted mid-session by a smug Alistair, who berated Luis for stringing along Pretty, Sheridan and Fancy. Luis said he wasn't stringing anyone along; he was simply doing what Alistair commanded them to do. Alistair continued to twist the situation around and portray Luis as a user and womanizer. Pretty angrily jumped to Luis' defense. Alistair accused Pretty of falling in love with Luis, too, and called her pathetic while holding them at gunpoint. He berated Pretty for being ugly and hideous, and told her she'd have to pay for sex for the rest of her life. Luis asked Alistair how he could hurt his granddaughter like this. When Alistair taunted Luis about Marty, Luis lunged for Alistair's gun and they struggled for it while Pretty looked on in horror.

At Harmony Hospital, Gwen was administering holistic remedies to Jonathan while Ethan remained skeptical. Ethan tried to give Gwen a reality check. Gwen continued to have faith in the holistic treatments. The doctor came to examine Jonathan. Ethan spoke to him privately and expressed his frustration at the situation since he's not the legal parent of record. The doctor told Ethan there is no hope for Jonathan outside of a liver transplant, and advised Gwen to let her stay in denial because of her fragile mental state. Theresa explained to Little Ethan how Jonathan needs a partial liver transplant and he's the only compatible donor available. Little Ethan expressed his fear of getting cut open. Theresa tried to reassure Little Ethan. He told Theresa he wants to help Jonathan but he's afraid. Then the child asked Theresa how he could be Jonathan's best donor if he's not related to Jonathan by blood. Theresa debated what to do, then said to Little Ethan there's something she's been wanting to tell him for a long time: Jonathan is Little Ethan's brother, because Julian isn't his father — Ethan is. Little Ethan told her how excited he is that Ethan is his dad, but asked Theresa why she never told him sooner. Theresa told him things were complicated, and she'd explain it all later. Theresa swore Little Ethan to secrecy. When Ethan walked in, Little Ethan ran over and hugged him, exclaiming "Daddy!" as a puzzled Ethan looked at Theresa.

Alistair continued to taunt Luis while Luis roughed him up. The Crane challenged Luis to kill him, promising, "Then you'll never find Marty!" When Luis tried to strangle him, Alistair zapped him with his electrocuting watch. "Luis is dead!" Pretty screamed as she hovered over his lifeless body on the ground. Alistair kicked Luis three times to rouse him and told Pretty she and Luis can rot there before leaving. Pretty splashed water on Luis to wake him up. When he came to Pretty started kissing him, screaming, "Thank God you're OK!" Luis didn't have any memory of his tussle with Alistair, but he did produce a key he pulled off of Alistair while they were struggling. Luis got dressed before they made their escape, while Pretty fantasized about a life with Luis ("I know I can make him love me," she said to herself). When Luis was prepared to leave Pretty told him she didn't want to go.

On the waterfront, Alistair was perturbed that Spike kept him waiting for their meeting. Alistair told Spike he had a "clean-up job" for him at the warehouse: Kill Luis — and Pretty, too, if she gets in the way.

Gwen had a nightmare where Jonathan died and everyone blamed her for his death. When Gwen woke up she came to her senses and realized the baby needed a liver transplant after all. Gwen broke down to Rebecca about how she hasn't been helping Jonathan this whole time. She wanted to go have Little Ethan donate part of his liver, but Rebecca urged Gwen to think things through and consider the risk of losing Ethan. Gwen insisted she had no choice. Rebecca blocked the doorway to their room and refused to let Gwen leave until she promised not to pursue using Little Ethan as a donor. When Gwen tried to push Rebecca aside, Beccs injected her with a sedative. Rebecca helped Gwen back into her chair. After passing out Gwen had another dream — this time she was attending Jonathan's funeral with Ethan. In her dream Gwen told Ethan she couldn't handle the service, then said she wanted to speak to Theresa. Gwen expressed her regrets to Theresa, who urged her to make their peace with each other and say goodbye to Jonathan together. Gwen woke up and found Ethan in the room tending to Jonathan. "I have something urgent to tell you about the baby," Gwen moaned as she tried to wake herself up. "We have a donor — we have a match."

Ethan asked Little Ethan while he called him "Daddy" and not "Uncle Ethan." "Mommy told me everything," said Little Ethan, as Theresa looked on in panic. Little Ethan looked to Theresa for an answer. Theresa clumsily covered and said she was trying to tell Little Ethan that he could be a comfort to Ethan if anything happened to Jonathan, "since they're all a family." Ethan sent Little Ethan off to the cafeteria for a snack. Theresa told Little Ethan on the side that he can't eat anything before his surgery. Ethan told Theresa how disgusted he was by Theresa's thought that Little Ethan could "replace" his son if anything happened to Jonathan. "That was the most appalling thing to ever come out of your mouth," Ethan angrily declared before walking out. Little Ethan returned and asked Theresa if he will ever be able to tell Ethan he is his son. "Someday," Theresa promised. "I just don't know when."

While Paloma waited for Noah to meet her, Noah came disguised as Santa and told her to sit on his lap. Paloma told "Santa" she was in love with her fiancé, but she'd had her doubts and wanted to know if she could trust him. "I can tell you exactly how your fiancé feels, Paloma," Santa said. Paloma was shocked that the Santa knew her name, and she asked him who he was. Santa didn't reveal his identity, but told Paloma her fiancé loved her more than anything in the world. Paloma told him she'd caught her love in bed with another woman, and she didn't think she'd be able to handle being disappointed again. Santa told her she didn't have to worry. Paloma thanked Santa, and when she walked away, Noah removed his beard just in time for Paloma to return and catch him. She began to cry, saying she couldn't believe Noah would trick her. She slapped him and asked why he was always trying to fool her. Noah tried to convince her that he loved her, but Paloma wouldn't believe him.

Back at Alistar's makeshift lair, Luis was ready to break out, but Pretty didn't want to return to the real world where everyone thought she was a freak. Luis told Pretty she had to trust him. "We can't live our lives down here," he said. Pretty thought about how she couldn't give Luis back to Fancy, and she refused to leave. Before he could convince her to escape, Spike jumped out and grabbed Pretty, holding her at gunpoint. Spike told them Alistair sent him to kill them both. Luis grabbed Spike and wrestled the gun away from him. While they fought on the ground, Luis told Pretty to grab the gun. Pretty got it and threw it to Luis. While Spike struggled to get it back, the gun went off and Pretty was shot.

At Harmony Hospital, Ethan told Gwen how much they'd been through and how hard it was to accept that they didn't have a donor for Jonathan. Gwen told him that he didn't have to worry because they had a match after all. Ethan couldn't believe it, and he asked Gwen who it was. She pretended to be too heavily sedated to respond, and when the doctor came in to tell them the good news, Ethan wondered how Gwen already knew about the donor if the doctor was mentioning it for the first time. Ethan finally decided Gwen must have dreamed that they'd found a donor for Jonathan while she was sleeping. He asked the doctor who it was, and the doctor replied that the donor wished to remain anonymous.

Meanwhile, Little Ethan prepared to donate part of his liver. Theresa, in tears, told him how brave he was. Little Ethan told Theresa he couldn't wait for the day when he could tell Ethan he was his son. With a smile, Little Ethan went into the hospital room, and Theresa sobbed, knowing Ethan would never know the secret and Gwen would never let her be with Ethan because of Pilar's past.

When Jonathan went into surgery, Ethan told Theresa they'd found a donor, and she pretended to be surprised. She then went into Gwen's room and told her she had to tell the truth so Jonathan wouldn't die. Gwen jumped up and informed Theresa she could make one phone call to Mexico and her whole family would be dead. Theresa said she only told Little Ethan and the doctor, but Gwen worried that Little Ethan would never be able to keep quiet. Theresa insisted the secret would be safe, and reminded Gwen that she should be thankful that Theresa was saving Gwen's son's life. Gwen sneered that she'd never thank her because saving Jonathan was another one of Theresa's schemes to trip Gwen up. In the middle of their fight, Gwen fainted. Theresa put a pillow under her head and walked out.

Later, Ethan begged the nurse to let him meet the donor family. At first she resisted, but she finally agreed to take Ethan to meet the mother of the donor child if he promised not to tell anyone. When the nurse took Ethan to the donor mother, he saw it was Theresa.

Fancy couldn't believe Luis slept with Pretty. While she thought about how wrong he was, she ran into Sheridan who was looking for him as well. Fancy told Sheridan she could have Luis after Pretty was done with him. Sheridan asked what Fancy was talking about, and Fancy showed her the video of Luis and Pretty making love. Sheridan didn't believe her eyes, and she asked where it came from. Fancy said she didn't know, and Sheridan said the video was probably a trick from Alistair. Sheridan said they needed to find Luis because he was probably in trouble, and she asked Fancy how she could be mad at Luis when she'd slept with Noah. "You don't love Luis," Sheridan said. "You never have." Fancy told Sheridan she was out of line, but two finally agreed to put their differences aside so they could find him. Moments later, Spike appeared and they overheard him telling Alistair how he'd taken care of Pretty and Luis. The both grabbed Spike and demanded he tell them where they could find Luis. Sheridan pulled Spike's gun on him, and said she'd shoot him if he didn't.

Pretty struggled to remain conscious as her shot wound began to take its toll on her body. Luis begged her not to die, and when he tried to move her, she screamed in pain. Luis said he would make Spike pay, and Pretty couldn't believe her own grandfather would send someone to kill her. Luis checked the wound out and discovered it was only flesh deep and told Pretty to keep applying pressure to it. Luis wanted to go get help, but Pretty asked him to stay with her a little longer before he went back to Fancy. "Do you think a man like you could ever love a woman like me?" she asked. Luis told Pretty she was an amazing woman and any man would be lucky to have her. Pretty asked Luis if he would sleep with her again. Luis told her he'd only slept with her because they were forced.

At the hospital, Gwen lay on the floor having a bad dream about Ethan being outraged she didn't tell him about Little Ethan. When she woke up, she realized Jonathan was in surgery and she rushed to go check on him. In the waiting room, Ethan asked Theresa why the nurse brought him to her when he asked to see the donor mother. Theresa asked if the nurse said she was the donor's mother, but Ethan said the nurse only said the donor's mother was in the waiting room and didn't point out Theresa specifically. Theresa told Ethan the donor's mother had been there earlier but left. Ethan was sorry to have missed her. He saw that Theresa was worried about Jonathan, so he apologized for the argument they'd had earlier. While they hugged, Gwen came in and asked Theresa what she'd said to him. Pilar told her she must be misunderstanding something, and Gwen apologized for overreacting. Gwen then asked Ethan to forgive her for not bringing Jonathan to the hospital sooner, but Ethan said he didn't blame her.

Ethan left to go find the donor's mother, and once he was gone, Gwen told Pilar she needed to help Theresa keep her mouth shut or else she'd expose their family's secret. When Ethan returned, he and Gwen consoled each other. Theresa cried to Pilar, wishing she could tell Ethan what she was going through. "I want to tell him," she said, "because every day I don't, a little bit more of our love dies." Theresa begged her mother to let her tell Ethan the truth, but Pilar told her she couldn't risk it because their whole family would die. Pilar also revealed that the people who were after them had already killed Theresa's aunt. "I'm so sorry," Pilar concluded, "but your dream of a life with Ethan is over."
